How to Refer a Patient
University of Minnesota Physicians has made it easy for you to refer patients to any of our physicians at University of Minnesota Amplatz Children’s Hospital through our Physician Referral Service. Through this service, you can schedule patient appointments or request consultations with our specialists.

There are two easy options for making a referral:

1. Call 888-KIDS-UMN (888-543-7866) to speak to a representative of our referral service.

2. Submit a request for a consultation or referral online.

We value our relationship with you, your patients and your office staff, and work hard to keep you informed of your patients’ care. Our goal is to provide you with prompt service and thorough communication about the patients that you refer to us.
